New Student Enrollment

NEW STUDENT ENROLLMENT

If you live in the Buckeye Central School District and have a student (s) in grades K-12, please follow these steps for enrollment:

1.      Visit our Final Forms page at https://buckeyecentral-oh.finalforms.com/ to complete and sign your school forms online.

2.      Schedule a registration appointment with our Enrollment Coordinator, Amanda Pace, by calling (419) 492-1024. This appointment MUST be done in person. Appointments are scheduled to expedite the process and to prevent parents/guardians from having extended wait times.

3.      When you arrive for your appointment, enter through the front High School doors. You MUST bring the following documents with you to your appointment:

          a.      Child’s birth certificate or hospital birth record – an original (or certified copy) must be presented

          b.      Parent/Guardian driver’s license or state-issued ID card

          c.      Proof of Residency

                                i.     Home Owner: One of the following – utility bill less than 30 days old, tax bill, mortgage statement, or homeowners’ insurance 

                                                                    statement/declaration.

                               ii.     Tenant: One of the following - Signed lease agreement with property owner or notarized Residency Affidavit

          d.      Custody papers, if applicable

          e.      Guardianship papers, if applicable

          f.       Student’s immunization records

          g.      Latest IEP/ETR/504, if applicable

          h.      HIGH SCHOOL STUDENTS: transcript from previous school

 

4.      The building secretary will contact you once everything is finalized and a start date has been set

 

Please note: Your child will NOT be able to attend until the school has ALL of the required documentation. It is a parents’ responsibility to obtain this documentation. Please do not expect the school to obtain this for you or for your child’s previous school to send it to Buckeye Central.

 

*If you live outside the Buckeye Central School District and are interested in Open Enrollment for your student, you must first contact the Superintendent’s Office at (419) 492-2864. Open enrollment must first be approved by the Superintendent before a student can be enrolled. Once approved, you will follow the steps above. *

**If you are interested in your child attending Bitty Bucks Preschool, you must fill out the Preschool Registration Form found under the Parent Resources tab first. Once your child has been approved to attend by the Preschool Director, you will then follow the steps above for enrollment. **
